Job Description

In this role, you will provide skilled nursing care to geriatric patients in the community, develop patient-centered care plans, and ensure patient well-being in the Newark, DE area. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Develop and implement goal-oriented, patient-centered care plans.
- Prepare patients for discharge and ensure continuity of care.
- Must be IV proficient or willing to train and perform IV functions.
Requirements
- BSN required; ADN candidates must obtain BSN within three (3) years of employment.
- Current DE RN license or compact state license.
- 2 years of recent HomeHealth RN experience.
- Previous Home Healthcare and Med-Surgical experience preferred.
