Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse specialized in Home Care to join our team. As a crucial member of our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. You will be responsible for developing and implementing patient-centered care plans, ensuring efficient discharge planning, and maintaining IV proficiency.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Develop and implement goal-oriented, patient-centered care plans.
- Plan for the discharge of patients.
- Maintain IV proficiency and perform IV functions.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of start date.
- Current DE RN license or compact state license.
- 2 years of recent HomeHealth RN experience.
- Previous Home Healthcare and Med-Surgical experience preferred.
- IV proficiency or willingness to be trained.

Weekly Schedule: 5x8, Monday to Friday, rotating weekends.
Expected Hours: 20 - 40 per week
Work Location: In person
Pay Range: $32.00 - $44.00 per hour
