Rehabilitation Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments and provide direct patient care.
• Proficiency in EMR documentation (Hello Note experience not required).
• CPR Certification.
