Responsibilities

• Conduct home visit assessments of patients, providing direct care and evaluating rehabilitation potential.
• Document patient progress using EMR systems.
• Ensure patient safety and well-being during therapy sessions.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficiency in EMR documentation (experience with Hello Note is a plus).
• CPR Certification.
