Responsibilities
- Conduct home visit Physical Therapy assessments for referred patients.
- Assess medical conditions, functional capabilities, limitations, and rehabilitation potential.
- Proficiently document patient records using EMR systems.
Requirements
- Graduate from an accredited Physical Therapy program.
- Hold an unencumbered Physical Therapy license for New York.
- Possess CPR Certification.
